Anticoagulation services in the modern NHS

A commissioning resource pack has been developed by Anticoagulation Europe, with the input of a wide range of experts, to support the development of anticoagulation services fit for purpose in the modern NHS.

The pack’s authors emphasise the emergence of new anticoagulation treatments for some indications, which do not require regular monitoring like warfarin. “This change in the treatment options for anticoagulation provides an important opportunity to consider the most appropriate service design to provide value for money to the NHS and a good experience and high quality outcomes for patients,” say the authors.

Given the different management and commissioning arrangements for short and long‐term anticoagulation usage, the commissioning resource pack focuses on services for patients on long-term anticoagulation therapy, considering the following groups:

  • People with artificial heart valves
  • People who have had a heart attack or stroke
  • People suffering from atrial fibrillation
  • People at risk of deep vein thrombosis
  • People at risk of a pulmonary embolism.

The resource pack lays out five steps to assist commissioners in designing appropriate anticoagulation services for people who require long-term treatment:

  • Identifying need and understanding the patient population
  • Understanding current services
  • Developing the business case
  • Designing the service and implementation
  • Monitoring and incentivising quality.

For each of these steps, the a section of the pack set out actions commissioners should take and resources which will provide information or evidence to aid them in this service redesign process.
